What you were doing before bug occurred: fighting minotaur and lost right hand
Steps to reproduce: lose a limb (maybe occurs for both left and right hands?) and equip vastira
F9 Logs ( press f9 to view them in-game): nothing useful

Some items/weapons you can’t wield when you lose a hand or an arm. Vastira uses both hands to wield, so that was why you couldn’t use it.

vastira mainly uses the right hand to hold it at the lower area, so if the right hand is lost, vastira disappears, but with the left hand, it works just as fine
vice versa with spear, except it is mainly held on the left hand at the lower area
