I guess, but it’s neither very graphic nor does it encourage people to harm themselves (since they’'re utterly useless) (I doubt it has much impact on the rating, but you could just remove the blood particles or just make it fade to black when you use it, and disable the animation)
it’s fantasy-fied so it’s mild (and they’ve[1] never been shown actually killing people so far)
the sacrifices are never really shown on screen, so it’s fine (and killing castaways is probably the least graphic violence in the whole game, it’s so goofy)

there is no category for drugs in the maturity questionnaire (also insanity is not exactly pleasant, so I don’t think it’d count as encouraging that behaviour anyway)
