Insanity 2 and 3 can actually damage you, i once ate an insanity 2 pie and died within 3 seconds.
Its most likely unintentional.

I have a feeling the recent insanity damage was temporarily added so people couldn’t abuse the bug that let you eat an insanity meal then sail into the dark sea to avoid getting killed by T5 insanity
update: decided to sit around at Akursius Keep with stuff containing Cursed Mushrooms and compile a list on the wiki after a while of not seeing a list there (it’s not complete but I did get a ton of the quotes)
