Have you linked your discord to your forum account (and connected your discord to patreon)?
That might give you the title/tag if linking patreon didn’t work.
Just a guess since I don’t have a connected patreon yet still got the titles

i went on the patreon site, disconnected it from the ao forums, went back to the ao forums, reconnected it, and it worked
try disconnecting your account then reconnecting it
