Some group with this symbol(kinda similar to the insanity symbol, maybe related?) drowned a bunch of people in diseased acid water

Dude who killed a dragon in a cave, he also has a kickass sword. The place is filled with poison I wonder if thats from him, the dragon, or something else. He may or may not be the guy from the theory above.

Any thoughts on this stuff?
